Screening Tools

Dr Jerrie Bezuidenhout uses various Patient Based Outcome Screening Tools to assist in the assessment of patients.

Patient-Based Outcome Measures are questionnaires that healthcare practitioners use to see what patients feel their health and well-being are. These screening tools assist in the patient-clinician conversation. Your clinician can physically see how you currently feel your illness impacts different areas of your health and life. By completing these questionnaires yourself, you can express exactly how you feel without interference. 

Some of the questionnaires may be repeated during your treatment to show us a trend in your treatment.

Please note that these are one-sided questions. Your state of mind, frame of reference and intentions may impact your results. Dr Jerrie will discuss the results with you. These are not diagnostic tools but screening and monitoring tools. Thus, without clinical input, the results are neither comprehensive nor objective representations of your illness.

Dr Jerrie in a Nutshell Team will send you the link to the specific questionnaire that Dr Jerrie would like you to complete. 

Complete the assigned questionnaires before your appointment so Dr Jerrie can discuss the results with you.
